Observations & Corrective Actions
2-2D: .07(3)(b) During inspection, observed their was no paper towel in the men's restroom. Person in Charge supplied paper towels for the restroom. Each handwashing sink shall be provided with individual, disposable towels, a continuous towel system that supplies the user with a clean towel, or a hand drying device. *This is the second repeat violation on a consecutive routine inspection. A third repeat violation may result in permit suspension.
6-1D: .04(6)(i) During inspection, observed facility was not following their time control procedures. Observed sliced cheese and reconstituted onions did not have a begin or discard time stamp on them. Person in Charge had employee place the correct time stamp on food. "When using Time Control (TPHC) for a maximum of 4 hours: 1) hot foods shall start at 135°F (57°C) or above and cold foods shall start at 41°F (5°C) or below; 2) the food items shall be labeled with a start and discard time, not to exceed 4 hours; 3) foods not consumed before the discard time shall be discarded; 4) when combining ingredients, the start/discard time for the foods shall reflect the start/discard time of the earliest ingredient placed on time control; 5) written procedures TPHC shall be created in advance and made available upon the Health Authorities request."
15C: .05(7)(d) During inspection, observed all equipment had an accumulation of old food debris in between, inside and underneath them. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. *This is a second repeat violation on a consecutive routine inspection. A third repeat violation may result in permit suspension.
17B: .06(5)(k),(l) During inspection, observed an accumulation of debris outside of the dumpster. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean.
17C: During inspection, observed tile missing in the front lobby by the front window. Observed a build-up of a black-like substance on the wall at the walk-in freezer entrance. Observed a ceiling tile in disrepair by the grill station. Hole in the tile was stuffed with unapproved material. Observed a leak in the ceiling outside of the office door. Observed an accumulation of old food debris on the floor throughout the kitchen. Observed the outside storage unit had an accumulation of debris on floors and throughout the unit. Observed old equipment not in-use. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
