Observations & Corrective Actions
1-2A: .03(1)(a)-(c) The PIC could not name all of the major food allergens. The PIC was educated during the inspection on the major food allergens and cross contact prevention. The PIC shall be knowledgeable of the major food allergens.
1-2A: .03(2)(a)-(o) The PIC is not performing their duties and responsibilities to ensure food safety rules and regulations are in compliance. The PIC was educated and trained on having active managerial control. The PIC shall have active managerial control at all times.
2-2A: .03(1)(c)2,3 and .03(1)(c)17 The PIC could not name all of the reportable symptoms and diagnosed diseases. The PIC was informed of the reportable symptoms, diagnosed diseases, restriction, exclusion, and the PIC's reporting requirement. The PIC shall be knowledgeable of their employee health policy.
2-2A: .03(4)(a)(b)(c)(e)(f) The PIC could not provide documentation of employee health training for selected employee. The PIC shall ensure all employees have been trained on their employee health policy via documentation.
2-2D: .07(3)(a) The hand sink in the kitchen was not supplied with a cleanser. The PIC supplied the hand sink with soap. All hand washing sinks shall have a cleanser available at all times.
2-2E: .03(6) The PIC was unable to provide an approved sanitizer (unable to determine concentration of bleach) for their clean up procedure for vomit/fecal events. The PIC shall ensure an approved sanitizer (bleach) is on-site.
4-1A: .04(4)(c) Observed raw container of chicken stored above multiple bottles of sauces inside the prep top cooler, observed mushrooms stored in the same container with raw shelled eggs, observed raw beef stored over cut onions in the prep top cooler, observed raw shelled eggs stored behind vegetables in the prep top cooler, observed cases of fries stored directly on top of boxes of raw chicken in the walk in cooler. The PIC stored foods properly. Food shall be separated and protected from cross contamination.
4-2B: .05(7)(a)1 Observed mold-like substance build-up inside ice chute and nozzles of drink dispensers. Food contact surfaces shall be clean to sight and touch.
6-1A: .04(6)(f) Cold held TCS foods in the walk in cooler were above 41F, see temperature log. Cheese was discarded. Raw wings and eggs were relocated to secondary cooler and freezer. Cold held TCS foods shall be kept at 41F or below.
10D: .04(4)(d) Observed multiple squeeze bottles that were not labeled with a food name. Food that has been removed from it's original container and is not easily identifiable shall be labeled with a food name.
15A: .05(6)(a) The walk in cooler is not maintaining an ambient air temperature of 41F or below to properly cold hold TCS foods. Cooler temperature 45F. Equipment shall be maintained in good repair and proper adjustment.
15C: .05(7)(a)2,3 Observed prep sink, 3 compartment sink, rice cooker, grill, underneath grill, rice cooker, vent hood, fan guards inside the walk in cooler, and reach in cooler soiled with old food debris and grease. Equipment shall be cleaned at a frequency to preclude accumulation.
17C: .07(5)(a),(b) Observed the following areas soiled with old food debris: wall behind prep sink and 3 compartment sink, pass-thru ledge, floor in the main kitchen, floor inside the walk in cooler, dining area floor. The facility shall be cleaned a frequency to preclude old food debris/trash build-up.
17C: .07(5)(f) Observed multiple wet mops not hung to dry after use. The PIC inverted mops to allow drying. Mops shall be hung to allow drying after use.
17D: .07(3)(h) Observed employee food stored above restaurant food inside the reach in cooler and walk in cooler. The PIC stored items correctly. Personal items shall be stored properly.
18: .07(5)(k) Observed flies throughout the main kitchen. Insects shall be minimized and controlled.
